A running record of every fieldwork observation ended under the disengagement rule, kept by the Field Safety Officer.
Disengagement invocation log#
Every invocation of the disengagement rule is logged here, per policy, regardless of outcome — including invocations that turn out, on review, to have had nothing behind them. The log exists so a pattern across observers is visible to the Field Safety Officer even when no single invocation looks unusual on its own.
| Entry | Date | Site | Trigger | Outcome |
|---|---|---|---|---|
| DL-01 | 2026-08-14 | West stairwell, Design building | Observer reported no longer feeling able to leave at will, ninth step of a descent | Observation ended immediately by buddy. No injury. The rule’s wording was rewritten the following week from a judgement-call trigger to the current procedural one. |
| DL-04 | 2026-09-30 | Seminar room B, Arts building | Buddy invoked on the observer’s behalf after a prolonged non-response to a scheduled check-in | Observation ended. Logged as a near-miss for the buddy-system rule rather than the disengagement rule itself; the observer had simply lost track of time. |
| DL-07 | 2027-02-24 | Printer room, Level 2 library | Observer invoked without a specific triggering event, citing only “not wanting to be the only one who knows why” | Observation ended. Retained as the clearest example on file that the rule does not require the observer to justify the call in the moment. |
